Track Your Form-76 Status

Enter your assigned Ticket ID and last name to view your current Form-76 eligibility review status. This tracker displays three items only: employment verification status, training verification status, and overall Form-76 status. If you recently submitted payment, allow a short processing window before checking status. If your Ticket ID is not found, verify the spelling of your last name and try again.

Once you submit your full Form-76 application, your listed Employer and Training Center contacts are automatically emailed from eot@govsource.net or from GovSource Public Safety with secure verification links. Applicants are strongly encouraged to contact their Employer and Training Center directly to confirm receipt of the verification email and to ensure spam or junk folders have been checked at the time of submission.

Follow-up with your listed points of contact is your responsibility. Eligibility review cannot proceed until both verification links are completed and all required documentation has been received. Once both verifications and required documents are completed, Form-76 issuance is typically finalized the same day or next business day.
