Form 76 – Start Your Equivalency of Training (EOT)

Application
Fill out the GovSource Form 76 application and submit payment securely online. This begins your Equivalency of Training (EOT) process with FDLE.
Use your full legal name and make sure all contact information is accurate (email, phone, and mailing address).
PDFs
After we confirm your application, a GovSource representative will email you two required PDF forms:
- Employer Verification Form – must be completed and signed by your former agency.
- Training Academy Verification Form – must be completed and signed by your training academy.
You will be responsible for forwarding these forms to the appropriate contacts.
Submit
Return both completed and signed PDFs to GovSource by email. To avoid delays, double-check that all sections are filled out and that your agency and academy use the correct contact details (names, titles, phone numbers, emails, and fax numbers if available).
Processing
Once we receive your signed forms, GovSource will:
- Review for accuracy and completeness
- Verify agency and academy signatures
- Submit your Form 76 packet to FDLE on your behalf
We will notify you immediately if any corrections are needed.
Approval
FDLE will review your application and issue approval once requirements are met. When approved, you will be officially eligible to enroll in the Florida Equivalency of Training (EOT) Course at a certified training academy.
