Understanding the CJSTC Form-76 Process
CJSTC Form-76 is not submitted directly by applicants.
Only an FDLE-approved assessment center may evaluate eligibility and generate Form-76 under Florida’s Equivalency of Training (EOT) process. GovSource operates as an FDLE-approved selection and assessment center authorized to review eligibility documentation and issue Form-76 for qualified applicants. The process begins by joining the GovSource Form-76 / Florida EOT Interest List.

Step 1 — Submit the Form-76 / EOT Interest Request
Applicants begin by submitting the GovSource Interest List. This short form collects basic background information including:
- Your qualifying law enforcement, corrections, federal, or military agency
- The state where your certification occurred
- Your employment background
- Your preferred Florida region for employment
Submitting the interest form allows the GovSource Selection Center to review your background and provide instructions for opening your eligibility review file.
Step 2 — Open Your Eligibility Review File
After submitting the interest form, applicants may open their CJSTC Form-76 eligibility review file. Opening the file requires submission of the eligibility review request and assessment fee.
Once the file is opened, applicants will receive an automated email containing:
- Instructions for assembling the required documentation packet
- The CJSTC-58 Background Information Form
- Employment Verification Form
- Training Verification Form
- Documentation submission instructions
Documentation cannot be reviewed until the eligibility review file has been opened.
Step 3 — Assemble and Submit Your Documentation Packet
Applicants must submit a complete documentation packet for eligibility review.
Required materials typically include:
CJSTC-58 Background Information Form (Notarized)
The CJSTC-58 must be:
- Fully completed
- Signed
- Properly notarized (any U.S. state)
A CJSTC-58 that is not notarized cannot be processed.
Academy Training Records
Applicants must provide documentation confirming completion of their basic training program.
Acceptable records may include:
- Academy transcript
- Certificate of completion
- Hour-by-hour breakdown of training (if available)
Incomplete or unreadable records will delay eligibility review.
Employment Verification
Applicants must provide verification of qualifying sworn employment. Employment verification is typically completed through the GovSource Employment Verification Form, which must be completed by the employing agency. Applicants are responsible for ensuring that the agency contact information provided is accurate.
Step 4 — Eligibility Review
Once a complete documentation packet is received, the GovSource Selection Center conducts an eligibility review. Eligibility determinations are based on:
- Verification of sworn employment
- Confirmation of academy training completion
- Review of submitted documentation
If additional information is required, applicants will be notified.

What Happens After Your Application Is Reviewed?
After documentation review is completed:
- The assessment center determines eligibility
- If qualified, CJSTC Form-76 is generated and issued
Once Form-76 is issued, applicants receive an official notification email containing access to the issued Form-76. Obtaining Form-76 confirms eligibility under the EOT pathway and allows you to:
- Register for the required in-person EOT skills training
- Upon successful completion of training, schedule and take the Florida State Officer Certification Examination (SOCE)
Issuance of Form-76 confirms eligibility. Final Florida certification requires completion of required training and successful passage of the SOCE examination.
